  2. Breastfeeding woman with an infant under 12 months
  3. Non-breastfeeding woman up to 6 months of postpartum
  4. Infant under 12 months of age
  5. Child 1 to 5 years of age
  6. Individuals with children participating in Medicaid, TANF, and free or reduced-price school lunch
  7. Note: Fathers, custodial grandparents, foster parents or other guardians may apply for WIC for their children.

  To begin enrollment, please bring the following to your first appointment:

  • Identification for all participants (Birth Certificate, Driver’s License, Social Security Card or Medicaid Card)
  • Proof of residence
