Promotes the health of individuals and families in the community through the development and implementation of
community health services. Conducts disease surviellance, program development, and education.

HOMEMAKER PROGRAM
In home housekeeping services to senior citizens, 60 years and older who have one or more debilitating
conditions, no transportation, and live alone.
COST: Voluntary donation accepted.
RURAL SCHOOL NURSING
Provides health education and screenings to children in the rural public schools that enhance a child's potential for learning.

Immunizations- children 2 months thru 18 years of age.
COST: Voluntary donation
HOURS: 1st and 3rd Thursday

SERVICES: Nutrition vouchers, education for pregnant women, infants and children to age 5.
ELIGIBILITY: Financial and nutritional guidelines.
COST: none.
PUBLIC HEALTH HOME VISITING PROGRAM
1. PREGNANCY SERVICES:
Education and referral services per R.N., Registered Dietitian, Social Worker, for pregnant women; to work
towards decreasing the incidence of low birth weight births and other poor pregnancy outcomes.
2. INFANT FOLLOW-UP PROGRAM
Follow-up and home visits to new mothers and mothers of children with medical and/or developmental disabilities.
Education and anticipatory guidance provided. Client or provider referral.
